Responsibilities:

  • The Clinical Coordinator LVN is responsible for scheduling, overseeing, and coordinating clinical services provided to patients receiving home health care.
  • The Clinical Coordinator will work closely with the Clinical Branch Manager/Administrator, clinical team, including registered nurses, therapists, and aides, to ensure that patient care is delivered effectively, efficiently, and in compliance with all applicable regulations and standards. 
  • Coordinate the scheduling of clinical services, including nursing visits, therapy sessions, and other treatments.   
  • Ensure that services are delivered according to the care plan and in a timely manner.   
  • Maintain accurate and complete patient records, including assessments, care plans, progress notes, and any changes in patients' conditions.   
  • Ensure that all documentation meets regulatory and agency standards. 
  • Ensure that all clinical services and documentation adhere to state, federal, and agency regulations, as well as industry standards.   
  • Stay updated on changes in regulations and implement necessary changes. 
  • Collaborate with physicians, therapists, social workers, and other healthcare professionals to develop and implement a holistic approach to patient care.   
  • Maintain open and effective communication with patients, their families, and healthcare providers agency management and staff.   
  • Address any concerns or questions and act as a liaison between all parties involved. 
  • Other duties as assigned.

 

Requirements:

  • Current LVN license in the state of practice.   
  • Previous experience in Home Health care is preferred.   
  • Strong clinical assessment and care planning skills.  
  • Excellent communication and interpersonal skills.  
  • Knowledge of healthcare regulations and compliance requirements.   
  • Basic computer skills for documentation and record-keeping.   
  • CPR certification.   

 

Job Knowledge:

  • Comprehensive knowledge of the function and organization of the home health agency. Knowledge of current rules and regulations in Home Care.
  • Ability to facilitate quality control issues. Comprehensive knowledge of general nursing theory and practice.
  • Ability to assume responsibility and effectively work with an interdisciplinary team.
  • Must be discreet in judgment and possess confidentially.
  • Ability to communicate effectively both orally and in writing, operate standard office equipment, read, learn and interpret complex rules and regulations, exercise tact and diplomacy, establish and maintain effective working relationships, gain cooperation from people over whom one has no authority: manage multiple interruptions while still completing projects and work assignments: maintain confidentiality of information.
  • Keep track of multiple projects simultaneously, evaluate information and make appropriate decisions, understand and solve problems within limits of authority.
  • Work under tight deadlines and maintain accuracy of work.
